FECT OF LOCAL RECURRENCE AFTER CONSERVATIVE TREATMENT ON THE PROGNOSIS OF PATIENTS WITH INITIAL INFILTRATIVE BREAST CARCINOMA

This research was done in order to evaluate if local recurrence (LR) after
quadrantectomy, axillary dissection and radiotherapy (QUART) in the treatment of initial
infiltrative breast carcinoma affects the prognosis of the patients. A total of 149 patients was submitted to QUART between 1981 and 1990 and was followed for a médium period
of 120.9 months (16 to 213). In 132 cases — group 1 - it was not observed any LR; in 17
cases of group 2, it was detected LR. In group 1, in 39 (29.5%) patients presented with
distant metastasis and 34 (25.8%) died; in group 2, it was verified 10 (58.8%) distant
metastasis and 9 (52.9%) deaths. The survival curves estimated by the Kaplan-Meier
method and analyzed by the log-rank test were statistically different for distant metastases
free survival (p = 0.03) and for overall survival (p = 0.01). The relative risk in patients with
LR after QUART for distant metastases was 2.09 and 2.34 for death. It was concluded
tliat LR after QUART aggravates the oncologic prognosis of the patients.
