Correlação anatomorradiológica de alterações mamárias através de core biopsy e punção aspirativa por agulha fina

Resumo

Durante o período de 8/3/1994 a 13/4/2000 realizaram-se 115 exames de diagnóstico por
imagem (mamografia, ultra-sonografia ou ambos) e foram indicados procedimentos intervencionistas em uma clínica de diagnóstico por imagem de Porto Alegre, no Rio Grande do Sul, a MamoRad, através de 44 core biopsiese 71 punções aspirativas por agulha fina (PAAF), orientadas
por estereotaxia ou ultra-sonografia. De acordo com a classificação do padrão breast imaging
reporting and data system (Bi-Rads) do American College of Radiology (ACR), 75 lesões foram
consideradas suspeitas (categoria 4) e 40, altamente suspeitas (categoria 5). A histopatologia
das core biopsies revelou que se tratavam de alterações malignas em 12 casos e benignas em
30; dois casos foram considerados inconclusivos, tendo havido solicitação de confirmação através
de biópsia excisional. A citologia das PAAFs mostrou alterações malignas em 23 casos e benignas
em 21; 27 casos foram inconclusivos, com solicitação de confirmação histológica. A comparação
da classificação dos exames radiológicos com os resultados anatomopatológicos permitiu obter
valores de sensibilidade (100%), valor de predição positiva de malignidade (27,94% e 74,35%)
e a acurácia (27,94% e 74,35%) das categorias 4 e 5 do Bi-Rads. Além disso, calcularam-se
sensibilidade (85,71% e 100%), especificidade (100% e 70%), valor de predição positiva de
malignidade (100% e 78,57%) e acurácia (95,45% e 84,1%) das core biopsies e PAAFs em
relação aos procedimentos cirúrgicos, possibilitando, desta forma, uma análise de desempenho
das biópsias. Os dados aferidos estão em consonância com a literatura médica.
