LINF0N0D0 SENTINELA: UMA ALTERNATIVA PARA 0 ESVAZIAMENTO AXILAR?

Resumo

aprimoramento das técnicas de detecção precoce do câncer de mama, principalmente por meio do rastreamento mamográfico, tem resultado no aumento do diagnóstico
de tumores em fase inicial, sem comprometimento dos linfonodos axilares. Teoricamente,
as pacientes não se beneficiam da linfadenectomia axilar completa, no entanto, um
acurado estadiamento do câncer de mama é essencial para orientar o manejo e determinar
o prognóstico da doença. A morbidade da linfadenectomia, quando pesquisada, chega
a 30%, justificando a necessidade de identificarmos previamente pacientes com axila
positiva e com indicação precisa de linfadenectomia. Mais recentemente, tem sido proposta uma alternativa à linfadenectomia axilar
nos estágios iniciais do câncer de mama, conhecida como pesquisa do linfonodo
sentinela (LFNS), o qual é definido como o primeiro linfonodo a receber a drenagem
linfática do tumor. O LFNS, identificado por intermédio de métodos combinados de mapeamento linfático com radioisótopos e corantes biológicos, seria capaz de predizer
o
"status" metastático da axila e talvez prescindir, em muitos casos, da linfadenectomia.
A proposta dessa revisão é discutir as vantagens e dificuldades encontradas
com essa técnica, como também demonstrar a associação de técnicas de mapeamento
linfático à biópsia do LFNS, permitindo o acurado estadiamento dos linfonodos axilares.
