Carcinoma Intraductal da Mama
Resumo
O autor tece considerações sobre o
carcinoma intraductal (carcinoma ductal
in situ) com referência especial aos
aspectos histológicos e as diversas
formas de tratamento desta neoplasia.
Reconhece a necessidade de dividir o carcinoma intraductal em vários
subtipos de acordo com o grau nuclear.
Dos carcinomas intraductaiso
comedocarcinoma in situé o que apresenta o pior prognóstico. Entre os
fatores que contribuem para a sua
maior agressividade estão o maior
pleomorfismo das células neoplásicas,
indice de proliferação celular, maior
possibilidade de associação com a
invasão oculta e a multicentricidade, o
elevado índice de aneuploidiaea
superexpressão da proteina neu do
oncogene HER-2/neu.
Os diversos subtipos histológicos
de carcinoma intraductal são descritos,
com particular referência ao
comedocarcinoma in situ, carcinoma
micropapilar, carcinoma de células aderentes, carcinoma apócrino com padrão sólido, carcinoma de células claras e carcinoma in situ com padrão de
células em" anel de sinête", carcinoma
cistico hipersecretor.
