AMAMENTAÇÃO E CÂNCER DE MAMA NA ZONA SUL DO RIO GRANDE DO SUL: UM ESTUDO DE CASO-CONTROLE

Vários estudos epidemiológicos colocam em controvérsia a relação do aleitamento
materno e o câncer de mama. Para investigar esta relação em nosso meio, utilizou-se um delineamento de caso-controle, com dois tipos de controles, emparelhados por idade. De
março de 1995 a julho de 1996, foram identificados 172 casos de câncer de mama em
mulheres de 20 a 60 anos, 516 controles hospitalares e 270 controles de vizinhança. As
principais variáveis estudadas foram a ocorrência da amamentação e o tempo da amamentação. Foi realizada análise multivariada, através de regressão logística condicional, para controle de fatores de confusão. Entre os casos, registrou-se uma
recusa e quatro perdas (2,9%); entre os controles, 14 recusas (3,5%). Não foi encontrado efeito protetor da amamentação para o câncer de mama. A razão de odds (RO) para quem amamentou foi de 0,8 (IC 95% 0,5-1,4) comparando-se com os controles hospitalares e de 0,5 (IC 95% 0,2-1,2) com os de vizinhança. Para as mulheres que amamentaram por seis
meses ou menos, a razão de odds foi de 1,0 (IC 95% 0,6-1,8) em relação aos controles
hospitalares e de 0,5 (IC 95% 0,2-1,2) aos de vizinhança. Nas mulheres que amamentaram
por mais de 25 meses, a razão de odds foi de 1,0 (IC 95% 0,6-1,9) e 0,5 (IC 95% 0,2-1,3) comparando-se com o grupo de controle hospitalar e de vizinhança respectivamente.
