LINFADENECTOMIA AXILAR SEM DRENAGEM DA AXILA EM CIRURGIA CONSERVADORA POR CÂNCER DE MAMA

No período de julho de 1996 a junho de 1997, 32 pacientes com 34 carcinomas
infiltrantes foram submetidos à ressecção segmentar de mama simultâneamente com
linfadenectomia axilar. Não se utilizou dreno após a dissecção axilar. Eram 31 pacientes
do sexo feminino e 1 do sexo masculino. A idade variou de 26 a 73 anos (média = 46). A
linfadenectomia foi total em 18 cirurgias e dos níveis I e II em 16. O procedimento
cirúrgico foi realizado com duas incisões em 21 casos e com uma incisão em 13 casos.
Todos os pacientes submeteram-se à aspiração de seroma axilar, variando de 1 a 17
punções (média = 6,3). A duração do seroma variou de 5 a 60 dias (média = 20). As
outras complicações da cirurgia foram: infecção, 4 (11,7%); deiscência parcial de sutura,
3 (8,8%); deiscência total de sutura, 1 (2,9%); e enfisema subcutâneo, 1 (2,9%) caso. Os
autores concluem que a linfadenectomia axilar em conjunção com a conservação de
mama pode ser realizada sem a drenagem da axila. O procedimento não aumenta a
morbidade e permite grande conforto pessoal ao paciente.
