O VALOR PROGNÓSTICO DA INVASÃO VASCULAR LINFÁTICA PERITUMORAL EM PACIENTES COM CÂNCER DE MAMA SEM COMPROMETIMENTO LINFONODAL AXILAR

Foi estudado retrospectivamente o valor prognóstico da invasão vascular linfática
peritumoral em 214 pacientes portadoras de carcinoma mamário, do tipo ductal invasivo,
sem comprometimento linfonodal axilar. As pacientes encontravam-se nos estádios clínicos I
(T1 NO) (94) e IIa (T2 NO) (120). Foram tratadas cirurgicamente através da mastectomia
radical e da quadrantectomia no Centro de Atendimento Integral à Saúde da Mulher (CAISM)
e na Clínica do Prof. José Aristodemo Pinotti (CLAP) entre 1980 e 1988. Nenhuma paciente
foi submetida à quimioterapia ou hormonioterapia, e todas as pacientes submetidas à
quadrantectomia foram também submetidas à radioterapia na mama operada. Estas 214
pacientes foram subdivididas em 2 grupos: Grupo I, pacientes que não apresentaram invasão
vascular linfática peritumoral (201) e Grupo II, pacientes que apresentaram invasão vascular
linfática peritumoral (13). Analisou-se comparativamente, em cada um dos grupos, a incidência
de recidivas. Após um período médio de seguimento de 8,1 anos, observamos 47 recorrências
(23, 8%) no grupo I e 11 recorrências (84,6%) no grupo II. Concluímos que as recorrências
são muito mais freqüentes no grupo II, e que a invasão vascular linfática peritumoral é um
importante parâmetro na avaliação prognóstica das pacientes com câncer de mama sem
comprometimento linfonodal axilar.
